Sorry to hear that you have been injured. There are some things you need to know and do.

  1. Start a journal; document what happened, who was involved, names dates, times, who you told, what was said; appointment dates, phone conversations, names and phone numbers of claims adjuticators, return to work specialists; how you are feeling, things you can't do.

  2. Contact your mortgage lender, vehicle financial lender, credit card company (s) , the sooner the better. Document this and what was said in your journal.

  3. Get a file folder started; keep track of ALL things related to your injury in one place. Write the date on each piece of information and number them consecutively as you collect them.

  4. If you haven't recieved a FORM 7 from your employer within a week, report it to WSIB. Companies are obligated to provide you with this information. When you get it read it carefully and make sure it is factual. If you find errors, contact your WSIB rep. or call your claims adjuticator.

  5. Complete the Form 6 , keep it specific to your claim, be truthful and be descriptive. Adjuticators probably have never seen what you do, which can make it hard to visualize

  6. Keep in contact with your employer, you are expected to co-operate by WSIB. Non compliance could cause problems.
